You are correct, it should not look like that. Not sure what I’m seeing there, but something is there that is causing the flat washer to deform. I think it would be a good idea for you to pull the bolt out check the bearing for damage and if it’s good replace the flat washer. Try to find a hardened flat washer.

It looks like the inner bearing race is broken. You will need a new bearing for sure. Hopefully the shaft is ok.

I don’t think it is the inner race, it is above the surface of the bearing. It’s something else.

Wow, Never seen that before, like something jammed under the flat washer and then bolt tightened. Maybe thread damage on bolt of shaft prevented tightening? Also. lot of peen marks on the outer surface on the bearing on high magnification. Also something I have never seen

Still think it’s the inner race. It looks like it has split in half and is now twisting itself out of round. You can see the balls are riding against it. Just my opinion based on what I’m seeing, but I bet as soon as he removes the bolt and washer it will be evident.

The inner race looks fine. Any chance someone was concerned about the bolt working off and then the bearing and gear coming off. Looks like a semi washer was put under the flat washer and the flat washer was bent over locking it?? But then there is a split washer on top that??

Looks like the bolt was too long and some lazy f just put a broken ring under it.

Looks like the wrong hardness of flat washer, in metric bolts you want a fender washer lock washer and bolt in the 10.9 hardness. Also check the depth of the threads on the counter shaft, I think the length of the bolt is fine.

Wondering if the other half of the ring washer is somewhere else in the engine?

You mean somewhere in the transmission? Unlike the engine there is a magnetic drain plug and most likely since the part is metal I doubt it would go up and engage with any parts.

There was multiple pieces of metal on the plug but the gears are otherwise in great condition.

Dang, foiled again by a small screen! I’m surprised that the ring used as a spacer stayed centered especially after it broke. You dodged a bullet, go buy a lottery ticket😀
